Essential Job Functions
| · | Oversee incoming referrals for Pathways, Summit and Village. Schedule Screenings, Intake and Tours, if applicable. |
| · | Oversee admissions for Pathways, Summit and Village. Confirm eligibility for the applicable programs and complete all admission documentation. Ensure that applicants entering Pathways have a Physician’s Authorization, and applicants entering Summit have a Recommendation for Rehabilitative and Tenancy Support Services. |
| · | Oversee discharges from Pathways, Summit and Village. Participate in Pre-Discharge and Discharge Conferences. Complete Discharge Plans. Oversee Follow-Up Services for discharged Pathways’ residents. |
| · | Responsible to comply with all Agency Policies including Agency Code of Conduct and Confidentiality Policies, Medicaid Compliance Program and Protocols and Justice Center Code of Conduct. |
| · | Required to be ambulatory and present and able to perform all job functions without limitations, up to and including life preservation activities. |
| · | Maintain attendance and punctuality. Notify Supervisor when late or absent. Provide adequate notice when scheduling time off. |
| Job Duties include but are not limited to: | |
| · | Act as information resource for TSLI administrative, supervisory and site staff. |
| · | Provide transportation as needed; must use own personal vehicle if agency vehicle is unavailable. |
| · | Reports to Division Manager. |
Full Time position, Monday-Friday 9:00am-5:00pm
Salary: Annual: $51,581
